The care manager works closely with your doctor, advanced practice nurse, allied health members and yourself to manage your chronic conditions. The care manager provides the following care to you:

• Obtain a thorough health history and assess your level of disease control

• Identify factors that contribute to your sub-optimal disease control

• Provide education on lifestyle modification and teach self-management to empower you to manage your health

• Monitor your condition and work with your doctor to review your plan of care regularly.
