1. Medical Claims Authorisation Form for Multiple Institutions [MCAF (M)]
The MCAF (M) allows the patient to use their own MediSave account only. If the patient wishes to use a family member’s MediSave account, they should approach our staff at the counter in our polyclinics to sign the MCAF (S). As long as the authorisation is not revoked, the MCAF (M) remains valid for an indefinite period and allows the patient to use MediSave at all Public Healthcare Institutions in Singapore.

2. Medical Claims Authorisation Form for Single Institution [MCAF (S)]
The MCAF (S) allows the patient to use either their own MediSave account or a family member’s MediSave account. This form permits the patient to utilise MediSave at the specific Public Healthcare Institution where the form is signed.
To sign the MCAF (S), please approach our staff at the counter in our polyclinics.

Important notes for Medical Claims Authorisation Forms:
Please kindly ensure you provide substantial identification documents for all parties notarised on the form (Patient / Additional Medisave Payer / party signing on-behalf of) for our necessary verification before we can submit the MCAF (S) / MCAF (M) on your behalf.
Minimum age for CPF account holder Medisave authorisation has been increased from 18 to 21 years old.
