NHG Healthier SG Primer 2.0 for GPs

​​​​​This course is designed to support General Practitioners (GPs) in acquiring the
necessary competencies to deliver high quality clinical care within the Healthier SG
framework and has been updated for 2024 care protocols. 

Consisting of asynchronous eLearning and a face-to-face interactive workshop, the refreshetraining comprises topics across Healthier SG’s different phases of care protocols, with a continued focus on multidisciplinary and holistic care for patients.​

Course Information

Ischaemic Heart Disease 

  • ​Comprehensively assess patients with diagnosed IHD
  •   Recommend optimal non-pharmacologic and pharmacologic management
  • Appropriate arrange for follow-up and provide the recommended care components
  • Know when to provide a referral to a specialist

Stroke

  • ​Comprehensively assess patients with stable stroke
  • Recommend optimal non-pharmacologic and pharmacologic management
  • Appropriate arrange for follow-up and provide the recommended care components
  • Know when to provide a referral to a specialist

Chronic Kidney Disease 

  • ​Evaluate a patient with chronic kidney disease including relevant laboratory investigations
  • Evaluate for reversible causes of CKD
  • Intensify and optimise pharmacology safely
  • Appropriate arrange for follow-up and provide the recommended care components
  • Know when to provide a referral to a specialist

Gout

  • ​Assess a patient presenting with monoarticular arthritis
  • Familiarity with the diagnostic criteria for gout
  • Provide long term care for patients with chronic gout
  • Recommend an individualised low purine diet for patients with chronic gout
  • Safely prescribe urate lowering therapy
  • Appropriately monitor for response and side effects of pharmacotherapy

Asthma 

  • ​Diagnose asthma and rule out other respiratory conditions
  • Assess asthma control using a validated tool and BREATHE factors
  • Manage asthma based on level of control in adults and children
  • Provide a written asthma action plan
  • Determine the severity of asthma exacerbation and manage accordingly
  • Know when to provide a referral to a specialist

COPD

  • ​Diagnosed COPD based on clinical presentation and spirometry findings
  • Comprehensively assess patients with COPD including evaluation of symptom burden and risk
  •  Manage patients with COPD holistically
  • Provide treatment for patients with acute COPD exacerbation
  •  Know when to provide a referral to a specialist
