  1. We offer you the options of changing or making an appointment online for the following services:
    • Antenatal Care
    • Mammogram
    • Pap Smear
    • Physiotherapy
    • Smoking Cessation
    • Dental*
    * The online request to change dental appointments is currently unavailable. Please call us at 6355 3000 if you wish to change your dental appointment.
  2. Our online appointment request is currently available for single-service visits. If you require two or more services, please contact us at 63553000.
  3. Please help us ensure that the new appointment date requested is at least 3 working days from the day of your online submission.
  4. We also seek your understanding that the eligibility to certain services will be based on medical criteria. These will be highlighted to you as you continue to the next page.
  5. Our customer service assistants will contact you via email within 3 working days.
